The PLS-5 requires a video of the assessment and a review form to be submitted to the National Evaluation Partners at FPG.

The video review form and any completed PLS-5 protocol can be emailed or shipped to Linda Soto at Frank Porter Graham Child Development Institute.

Digital Video Submission

Videos can be digitally uploaded via Google Drive or box.net

To submit via Google Drive, please use the following email address: fpg.educare.reliability@gmail.com

Please save as MP4 or mov file format and use the following naming convention:

For English assessments: ATL_PLS5_assessorlastname_assessorfirstname_(yyyymmdd).(mp4 or mov)

For Spanish assessments: ATL_PLS5spanish_assessorlastname_assessorfirstname_(yyyymmdd).(mp4 or mov)

Physical Video Submission

Alternatively, PLS-5 videos can also be shipped on a thumb drive via FedEx to the National Evaluation Partner.

Listing in Video Queue

Finally, please list your video on the Video Queue. 

The video queue lets FPG know you’ve submitted materials and then lets you see that we’ve received your materials (or that something is missing) and lets you check the progress of the review.  It also lets you see how many videos are already in the queue from other sites. Click List a Video and follow the prompts.  Then you can track your video’s progress!

**The generic educareuser login will not get you to the video queue.  If you have not received an email inviting you to set up your username and password for the video queue, email Gisele to request an invitation.
