Section B of our Study Tools and Forms page provides a site review form for each measure and guidelines for submitting videos and accompanying materials to FPG.  For each assessor who will collect PPVT and PLS data at your site, LEPs must use the following procedure to receive confirmation from FPG of correct adminsitration. 

After the assessor has been trained and has practiced, record a video of the assessor establishing a rapport with a child and completing the entire assessment. 

A member of the LEP team then reviews the video, completes a review form, and submits the video, completed protocol, and review form to FPG. 

The LEP adds a listing to the Video Queue for each video submitted.  Email Gisele Crawford for permission to use the Video Queue. 

FPG reviewers will complete the FPG section of the review form and send it back to the LEP, and will indicate update the Video Queue to show the status: Under Review; Review Submitted for Final Determination; Determination Sent to Site.
